IMPORTANT INFORMATION ABOUT LOCAL TELEPHONE SERVICE AND THE LIFELINE PROGRAM DISCOUNT FROM
CONSOLIDATED COMMUNICATIONS
Consolidated Communications of Minnesota Company, LLC is your local communications partner providing reliable, adaptable services at reasonable rates within its service territory. Basic services include voice grade access to the public switched telephone network or its functional equivalent; local minutes of use; access to emergency services where they have been implemented by local government agencies (governmental surcharges may apply) and toll limitation service for qualifying low-income consumers. Basic services are offered at the rates below. This notice is in compliance with 47 CFR 54.201(d)(2).
Monthly Service Charges
Single Party Residential Service $33.50
Federal Subscriber Line– Single Line Res $ 6.50
Single Party Business Service $70.00
Federal Subscriber Line and Access Recovery Charges – Single Line Bus $ 9.50
All rates indicated are as of August 1, 2025. Taxes, fees and surcharges are not included in the pricing above. Taxes, fees, surcharges and rates are subject to change.
Consolidated Communications offers a Lifeline Program for eligible low income residential subscribers at their primary residence. The Lifeline Program is a government benefit program. Eligible subscribers can apply a monthly federal Lifeline Program discount to an eligible service. If applicable, there is a Federal Tribal Lifeline Program which provides additional discounts. The state of Minnesota also offers an additional discount for eligible subscribers.
To qualify for Lifeline Program support, residential customers must receive benefits from qualified assistance programs. Alternatively, customers with household income meeting specific Federal Poverty Guidelines may also qualify for Lifeline Program support. Eligible subscribers who apply the Federal Lifeline Program discount to their voice service may also qualify for free toll blocking to help control long distance usage.
Only one Lifeline Program discount is available per household on either a wireless or wireline qualifying service. Under the Lifeline Program, a household is defined as any individual or group of individuals who live together at the same address and share income and expenses. The Lifeline Program benefit is non- transferable. Consumers who willfully make false statements in order to obtain the discount can be punished by fine or imprisonment or can be barred from the Lifeline Program.
